LEADERSHP PROJECT PLAN AND REPORT
The following information is for the Teen and Junior Leadership form as we use it in the County Leadership project. The intent of this memo is to provide assistance to Junior and Teen Leaders in the completion of the annual leadership plan and report. Information provided is meant to stimulate your thinking for your own plan. Work with your Leadership project leader toward coordinating all of your 4H leadership (in all projects/areas) into this one report. A new Leadership Plan and Report should be placed in your Record Book each year. This is your record of Leadership performed anywhere (Project, Club, County, Section, State, National are all shown on the one form each year. You could, if you wish, include some school leadership, etc., for your own information later.(Page 6). Pages and photos can be added as you wish.

- The first 3 pages are completed at the beginning of the 4H year.

- Pages 4 and 5 are best if kept up to date all during the year.

- Pages 6 and 7 are definitely meant to be completed at the end of the year.

- Page 7 contains a space for your Leadership Project Leader to complete comments.

If you are working with a specific project during the year, it is good to complete a project report form with your plans for each meeting during the year. Work with your Project Leader to complete this information and have the PL sign it as a Plan of Work. Insert this between pages 2 and 3 as a Plan of Work and have your Project Leader sign it again at the end of the year (or even a new form which shows what was actually accomplished). You could instead outline your plans on pages 2 and 3 if you wish. However, we do want ALL of your leadership activities to be included in your plans, not just one project. If you work with more than one project, repeat the process for each project working with that project leader.
